How to Choose a Nursing Home in the USA

Updated 30 October 2026 · Educational resource

Choosing a nursing home is a significant decision for individuals and families. Quality, location, staffing, and payment options all matter. A structured approach can help identify facilities that meet both clinical and personal needs.

Start with official quality information

Use the CMS Care Compare tool to review star ratings, inspection reports, staffing levels, and quality measures for nursing homes in your area. Look for facilities with recent survey results and note any deficiencies related to resident care, infection control, or staffing. State health department websites may also publish additional complaint or inspection data.

Visit in person

Schedule visits at different times of day. Observe how staff interact with residents, whether call lights are answered promptly, and the general cleanliness and atmosphere. Ask to see a typical resident room, dining area, and activity space. Speak with current residents or family members if possible.

Questions to ask

Also ask about costs, what is included in the daily rate, and whether the facility accepts Medicare, Medicaid, or private insurance. Payment rules differ by state and individual eligibility.

Disclaimer: This is general guidance only. Nursing home selection depends on individual health needs, location, and finances. Verify all details with the facility and official sources.
