Regulatory Oversight of Nursing Homes: US CMS and UK CQC

Updated 30 October 2026 · Educational resource

Long-term care facilities in the United States and the United Kingdom operate under different regulatory systems, but both aim to protect residents and ensure minimum standards of care.

United States

Nursing homes that accept Medicare or Medicaid must meet federal requirements set by the Centers for Medicare & Medicaid Services (CMS). State survey agencies conduct inspections, investigate complaints, and can impose remedies ranging from plans of correction to termination from the programs. Results are published on Care Compare.

United Kingdom

In England, the Care Quality Commission (CQC) registers and inspects care homes. It rates services and can take enforcement action. Scotland, Wales, and Northern Ireland have their own regulators with similar roles. Local authorities also have safeguarding and commissioning responsibilities.

In both countries, families can review public inspection reports, raise concerns with the regulator, and contact advocacy services such as the long-term care ombudsman (US) or local authority and voluntary organizations (UK).
